Who we are

The data controller is Four Gates Group Limited, trading as Pharmacy One, registered at 262 Finney Lane, Heald Green, SK8 3QD.

What we collect

When you submit the prescription request form we collect the fields you provide: full name, date of birth, phone number, optional email, GP surgery, medication list, delivery details where applicable, optional notes, and your consent confirmation. This data is treated as special category health data under UK GDPR.

When you submit the contact enquiry form we collect your name, email, and message.

How we use it

We use prescription request data only to process your prescription with your GP surgery. We use enquiry data only to reply to your enquiry. We do not share it with third parties for marketing.

How we store it

Form submissions are emailed to a pharmacy mailbox controlled by our pharmacist. We do not store form submissions in our own database. Our hosting provider (Cloudflare) retains access logs including IP address for up to 30 days under their standard policy.

Your rights

You can ask us for a copy of any data we hold about you, ask us to correct it, or ask us to delete it. Contact us by phone on 0161 437 2634 or using the enquiry form.
